Property Owner Information Request Sheet

The Property Owner Information Request form is utilized when the public want to know information on a specific property, including who the owner is. The requestor must fill out the correct information and email it to an OEWD staff contact. The requestor is only to fill out information highlighted in yellow. This includes the APN for the property or properties. APNs can be found by using the Property Information Map and typing in a property's address.

Please email completed Property Owner Information Request Forms to: CBD.Assessment@sfgov.org and in the subject line please write: Property Owner Information Request (indicate neighborhood), for example: Property Owner Information Request (Fisherman's Wharf).
